You can use this form to register your child for Grades 1-12 at any of our schools. If you are registering your child for Kindergarten, please click here to visit our Kindergarten website, K at KP, and use the Kindergarten Registration Form.

  • STEP 1: SCHOOL INFORMATION
  • While you fill out the form below, you may want to have your child’s personal information handy, as well as contact information for you and emergency contact(s). Let’s get started! To begin with, we need to know which school your child will be attending.

  • STEP 2: STUDENT’S INFORMATION
  • Here’s where you’ll provide us with basic personal information about your child, such as names, contact information, and medical information. Please remember that all of your information is secure and will not be shared outside of the school board and your school’s administration team.
  • Please note you will need to bring proof of your child's date of birth into the school to complete the registration process. Proof can be a birth certificate, passport, or baptismal certificate.

  • If your child has more than one address, please input the primary address. In Step 4 & 5 you'll have the opportunity to add alternate and additional addresses.
  • Please note you will need to bring proof of your address into the school to complete the registration process. Proof can be: current property tax bill, current home utility bill, current internet/cable bill, current agreement of purchase and sale. If none of these are available, please contact the school to discuss alternative options.

  • STEP 3: CITIZENSHIP INFORMATION
  • Your child’s citizenship informs us how his or her education will be funded.
  • Please note you will need to bring your child's Study Permit or Visitor Record into the school to complete the registration process.
  • Please note you will need to provide information about your child's exchange agreement to the school before your registration can be completed.
  • Please note you will need to provide documentation confirming your permanent resident status to the school before your child's registration can be completed.
  • Please note you will need to provide documentation confirming you and/or your child's refugee status to the school before your child's registration can be completed.
  • Please note you will need to provide documentation to the school (confirming your Study Permit) before your child's registration can be completed.
  • Please note you will need to provide documentation to the school (confirming your Work Permit) before your child's registration can be completed.
  • Please note you will need to provide documentation to the school (confirming your Diplomat Status or Minister's Permit) before your child's registration can be completed.
  • STEP 4: PARENT/LEGAL GUARDIAN INFORMATION
  • In Step 1 you provided address information for your child. In this step we’re looking for more detailed information about you—your child’s parents and guardians. If you need to provide additional emergency contacts for your child—alternate guardians, grandparents, relatives—you’ll have the opportunity to add more in Step 5.

  • STEP 5: ADDITIONAL CONTACT INFORMATION
  • Here we’ve provided space for you to include additional contacts on your child’s record. These contacts (alternate guardians, babysitters, grandparents) will be contacted by the school if/when the child’s primary contacts or parents cannot be reached during an urgent situation or emergency.

  • STEP 6: FINAL REGISTRATION INFORMATION
  • Almost done! Here we’d like you to indicate (for our records) if you’ll be arranging bussing for your child’s transportation to and/or from school. We’ve also included space for you to tell us anything else you’d like us to know about your child.
  • Bussing in the Keewatin-Patricia District School Board is arranged directly between the parent or guardian and the Northwestern Ontario Student Services Consortium. Please visit their website at www.nwobus.ca or contact their general inquiries line at (807) 223-1256 or Toll Free 1-866-860-7770.

  • The Ministry of Education and Training, under the authority of the Education Act of the Province of Ontario, R.S.O. 1990 Ch. E2, Section 265, requires that each school maintain a record of basic information for each student registered in the school. The information will be used to:
    • Document registration of the student
    • Identify any special needs of the student
    • Prepare the Ontario Student Record* and OSR office index card
    • Prepare class lists, attendance reports, and other reports for the Board and the Ministry of Education
    • Provide other school Boards with required registration information, if the student transfers to another school board
    • Supply the District Health Unit with information for public health records
    • Make emergency care arrangements for the student
    *The Ontario Student Record is a record folder containing information about your child including report cards and other personal information conducive to the improvement of the instruction of the student. Parents(s)/guardian(s) have the right to access or examine the contents of the OSR folder for their child who is under 18 years of age.

In accordance with Canadian Anti-Spam Legislation which requires all organizations in Canada to collect consent for the receipt of commercial electronic communications, we are informing you that by providing your consent below and your email address you agree to receive messages electronically from the school or school board which may include information about a commercial activity. I understand this consent will be effective for the duration of my child’s education at the Board. I understand this information may be shared with the School Council (co‐)chair(s) for my child’s schools for the purposes of sending School Council information to me via email. I understand that I may withdraw my consent and unsubscribe from the communications at any time by contacting the school.
